   It's basically a genetic engineering project that got out of control   
   and turned into a plague.  In "Affliction" we see a Klingon general and   
   a scientist overseeing experiments on a prisoner - they inject him with   
   a formula which makes him stronger but which causes his forehead to   
   shrink.   
      
   Later on, Enterprise is raided by a whole troop of TOS-style   
   (ridgeless) Klingons who are demanding to know what is happening to   
   their race.
