   I doubt you could find an ISP that hasn't shut off at least 1 person's   
   service for violating their Terms of Service.... after all that's one of   
   the reasons they have the ToS.   
      
   I remember a time when it was not so difficult to trace a spammer back   
   to their ISP (prior to the proliferous use of anon remailers and other   
   such tricks), and I know from first hand knowledge that several of the   
   spammers I reported to their ISP were disconnected for violating their ToS.
