   Their "inner makeup" is out of whack.  IOW, because they   
   have a mental health disorder.  Otherwise, their biology and   
   "inner makeup" would be in sync.   
      
   These people know something is out of whack, but they don't know   
   exactly is wrong with them. They try to fix it by behaving as   
   someone they're not.  IOW they self-treat a symptom while leaving   
   the underlying problem unfixed.  The simple fact, a biological   
   male can never actually know what it feels like to be a biological   
   female (and vice versa).  They haven't actually transformed like a   
   caterpillar into a butterfly.  It's all in their imaginations.   
   They're not "trans", they're "ersatz", an imitation, a caricature   
   of the real thing.
