On Comcast, they essentially give away basic cable with Internet
service. The list price is not what many people pay. You just go to a
Comcast independent salesperson who sets up a table in Fry's, Walmart,
etc., and they are able to advise you on how to get the best price. But
many people just pay the list price.

My mother-in-law watches one channel. Her Comcast bill was $85 per month
with no Internet. I changed it to a $45 per month plan with Internet
(normally $35 but the one Chinese channel she watches is $10 per month
   extra).

Both AT&T and Comcast offer fiber to the home, but it's expensive and
   unnecessary.
