   Agree that if it's a one-time affair with few pages, I'd just print to PDF   
   or JPEG and then edit in Irfanview (which "merges" well because it resizes   
   automatically to allow both pages to be the same edge size) or in Acrobat   
   (which appends pages well) or in Paint.NET (which does everything).   
      
   Note you can "temporarily" merge the long sides horizontally with   
   Irfanview, and then chop the two pages apart now that they're both the same   
   long size.   
      
   But if the OP has many pages, then that will likely be too much effort.   
      
   I hesitate to recommend Posterazor freeware but I'd explore that if many   
   pages are needed. Maybe it's time to revisit the PDF chart shown below?
