   Maybe because it doesn’t say that. It puts the defect “chiefly in the   
   parietal”, which is on top and side of the head. It merely adds that the   
   damage “extends somewhat”into other areas (occipital and temporal) but at   
   that point, it’s unclear    
   if the report means the skull bone, the scalp, or both.

   Note that the report references the “more or less intact scalp”, which is   
   what the photo you reference shows.
