   Usually, the psychotic stuff is so pervasively disabling that nobody even   
   bothers diagnosing anything lesser in addition.  They just treat all the   
   symptoms as if they arose from the same cause.  It ain't necessarily so, and   
   they know it, but it's easier that way.
