The nanites were what caused the problem.

   *SPOILER ALERT*

Jack appears in Blitz-era London with far-future tech... an ambulance. The
ambulance is peopled with nano-thingys that will repair damage on a
battlefield. They found the boy with the gas mask after he was hit, and
thought the gas mask was part of him. So, they "fixed" him, but with the
mask. Then, they proceeded to "fix" as much of London as they could (the
creepy people in the hospital). The Doctor parsed out that the girl was not
indeed the boy's sister, but his mother. The nano-thingys recognized the
mother's DNA as correct and re-fixed the boy, as well as all the creepy
people in London. Much joy abounds. We get hot omnisexual Jack in a WWII
uniform, cute Rose, and the Ninth Doctor with a happy ending. What's there
   not to like?
