Obi-Wan beat Anakin in Episode III. The Emperor had to come along and
rescue Anakin and rebuild him as Darth Vader.

Obi-Wan purposely lost to Darth Vader in Episode IV so that Luke would not
insist on staying to try to rescue him.

Part of the reason Obi-Wan lost to Count Dooku in Episode II was due to
   Anakin's recklessness.

General Grievous was a cyborg - part living being, part robot (having also
been rebuilt by the Emperor) and although not a Sith Lord, he was trained
in lightsabre techniques by Count Dooku.

Anakin, said to be the greatest Jedi ever, also lost to Count Dooku in
Episode II, but then beat him in Episode III. He also lost to Obi-Wan in
   Episode III.
