   (2)    Number of passenger's  (Children under 5 1/2 travel as half adult   
   weight).   
   (3)    Baggage weight.  (In kilo's please).   
   (4)    Aircraft type, single/twin engine.   
   (5)    Meals and refreshment requirements  (You're having non of mine)   
   (6)    Cabin staff required or self-service.   
   (7)    Long haul flight may require refuelling stop(s), airport handling   
   fee's and co-pilot.   
   (8)    Date of expected voyage, seasonal rates apply.   
   (9)    Open or closed cockpit.   
   (10)  Wings, including flaps and trim equipment, yes/no.   
   (11)  Tail assembly, yes/no.   
   (12)   Landing lights, yes/no   
   (13)   Navigational equipment including compass, pencil, pencil sharpener,   
   note pad for doodling and eraser (rubber).   
   (14)   Navigation lights. yes/no.   
   (15)   Landing/take off assembly, including wheels and tyres.   
   (16)   Cabin seating and seat belting, yes/no   
   (17)   Toilet facilities, luxury/standard/basic, yes/no  (*Note:  Basic
