   Cards go bad (and do have limited cycles), Ports get 'glitched' and lock-out.   
      
   You may be trying to fix a 'card' problem, when the Port the reader uses is in   
   trouble. or the drivers for the USB 'Root hub'  or Controller device   
      
   The fact you have USB problems may indicate something closer to the data bus   
   is having issues/ has gone bad.   
   Switch the device Manager View to "Devices by Connection" and you can see how   
   deep the nest can get.
