The coax signals come from a combination       from the CMTS (which handles data) and the video head end (which       produces both analogue and digital TV signals). Every thing is packed       into 6mhz chunks which correspond to the bandwidth for the old NTSC       analogue TV channels.              For most cableco FTTH deployments, they use RFoG which basically means       that you have digital signals converted to analogue RF and then       converted to digital light. At the home, you get a fibre to coax output       with analogue RF to the DOCSIS modem and your TV set top box which then       process 6mhz channels as data, analogue TV or digital TV.              For some FTTH cable deployment, they really use a GPON/ethernet system       and deliver IPTV system and no RF/coax compatible gear. This comes off       totally different equipment and has no analogue channels at all.              Telcos, not having any investment in RF equipment like the cable       companues will generally go the GPON route with IPTV. The big exception       is Verizon which opted for RFoG and GPON data.
