   Hi Claire,

I just received my fathers records after having ordered them in
   December. Go to this website:
   http://www.collectionscanada.ca/genealogy/022-909.007-e.html. All the
info you need is there. There is a simple .pdf application form that you
can download. I just faxed the form to Ottawa. It will take a few months
to receive the records because they are backlogged.

My condolences on the passing of your father. I can sympathize. My dad
passed away in 1998 and I'm still tying up loose ends.
