   Decades ago, I worked solving these kind of problems. But VoIP was not   
   yet in the picture. Had you asked this question back then, I would have   
   said that you have to report this problem to your provider customer   
   support to have it investigated. Some bad routing table somewhere; they   
   were complex and many exchanges could be involved. But the starting   
   point was known.
