Are you using grub's x86_64-efi target?  You might have to specify it with
"--target=x86_64-efi" to grub-install.  On mine it is now the default ... and
it works with only the additions to /etc/default/grub as previously shown.

No.  It happens before udev is mounted.  I'll probably return to this later
but for now I just have it booting to the nvme.  Just trying to keep it simple.
