   That happened once on a Chicago "L" train, between Belmont and Fullerton.   
   One door didn't close at the station, but the interlock failed and the   
   train took off at speed. The car wasn't densely packed, but it was SRO.   
      
   I sent a letter to the NTSB with all the details, cc: to the CTA, and   
   got a letter from the CTA denying that it had ever happened.
