I made it myself. 

Try https://outpostbbs.net/files/OS_2.rar (case-sensitive and hotlink
protected so make sure you type that into your browser).

You will have to fiddle with the networking on it with your system but I know
of at least three people that are using that image successfully.  It's stock
OS/2 Warp 4.52 with the OS/2 Virtualbox Add-ons installed.
