The front and rear of the locomotive are essentially just regular
locomotive frames. The pilot wheels are tied to these frames just like
they would be if they were separate locomotives.
Thanks to the exaggerated curves of Lego train track it is possible to
see the pilot and trailing wheels have moved relative to the separate
frame on the front and rear. However, they are still tied to those
front and rear frames, just as they would be in the standard locomotive.
There are no wheels at all tied directly to the middle section of the
locomotive (At least, there aren't in a real Garratt locomotive.) The
boiler and cab are all on a single frame, which is suspended between the
two locomotive frames.
The concept proved good enough that it was actually adopted for use by a
few electric locomotives, and these may show the concept a bit better:

Notice that the end of the locomotive is a separate frame, and the
center body is actually just tied to these above the axle that faces the
center of the locomotive. So, instead of one long frame it actually has
one somewhat shorter frame and one very short frames at each end.
On a Beyer-Garratt, those four wheeled end pieces are instead replaced
with an entire 4-8-2 or similar steam locomotive, and the cab and boiler
take the place of the middle.
