   Such idiocy by the higher ups happens. There are many, many examples   
   throughout human history.   
      
   As long as they have enough Government money to keep making new toys   
   and can sit safely back in the comfort of HQ a long, long, long way   
   from the actual fighting, the higher ups a perfectly happy and couldn't   
   give a damn about the troops.   
      
   Someone mentioned tanks in World War II ... you'll notice it's   
   extremely rare to find high ranking officer in a tank during an actual   
   a battle, or anyway where else even remotely nearby. They're usually   
   only seen in tanks during PR stunts.
