I had an aftermarket cellphone installed in my car
in 1990, so, that's the earliest that I remember.

But that thing was the size of a portable landline
handset, and, interestingly, it was hands free for
talking (the mike was run by a physical wire to just
   over the driver's door).

I don't remember what it used for a speaker, but, I
think the speaker was in the handset itself (IIRC).
