   could someone please explain me why does Boyd drop the indices on   
   \omega_s and \epsilon_T in equation 2.11.5?

   Also, what is that mix on \epsilon in 2.11.16 and 2.11.17?!   
   The last term, varying with z, in 2.11.17 has to be multiplied by   
   1/\epsilon_T, but I do not see how one would get 1\epsilon_T in 2.11.16   
      
   Any explanation is highly appreciated.
