   If a man and his wife have grandchildren by 2 of their children, the   
   grandchildren of each family are 1st cousins of each other.  There is no   
   blood relationship between your grandchildren and those of your second   
   wife so you could say they are step-1st cousins, but I don't know if   
   that is a commonly (or legally) used term.
