it never works all of the time and I don't know how to negotiate
through what you called a markless realm, but when I do it I focus
on the third eye area of both myself and the target person and try
to dwell upon the intent exclusive of other thoughts or feelings.
the times when I know someone is picking up on my thoughts
inadvertently I can usually block their reception once I see that
it is occurring, but I can't say how or why it works when it does
   work.

several people in my family have psychic abilities. my sister is
very good at reading other's thoughts. sometimes it's as if she
is hearing them speak out loud.
