   Like most fields of medicine, the drug and dose that works with   
   acceptable side effects vary from person to person.   
   And yes, it is certainly true that there is a bias against prescribing   
   opiates, an ironic one since these are among the safest drugs. I've   
   encountered it myself several times, even among specialists. The   
   propaganda of the DEA and others with a special interest in demonizing   
   drugs has affected even doctors.
